8/27/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax verbalized to QMHPA that her morning was going good. QMHPA informed Ms. Broadnax that this session was going to continue to focus increasing her independent living skills. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHPA began the session by inquiring about Ms. Broadnax current thoughts, feelings, and emotions. QMHPA discussed with Ms. Broadnax the importance of self-care. QMHPA explained to Ms. Broadnax it is good to be on a schedule to complete her ADL's and ILS's daily. QMHPA questioned Ms. Broadnax about completing a calendar with stickers and keeping the calendar on the wall without misplacing it. QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to assist Ms. Broadnax with the importance of being able to separate and place her clothes in proper storage. QMHPA engaged Ms. Broadnax in another skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to demonstrate with Ms. Broadnax proper storing and separation of clothing through role play. QMHPA explained to Ms. Broadnax good hygiene regiments and how to apply them daily. Response: Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax said she takes care of herself and sometimes she does not, she is not going to lie. Ms. Broadnax said she brushes her teeth and shower, but she does skip days because there are days that her mind is telling her not to complete certain things or she just do not feel like it. Ms. Broadnax said she does separate her clothes but will wear the same because her others clothes are dirty. Ms. Broadnax will be lazy at times and will not look for other clothes. Ms. Broadnax indicated to QMHPA that she just have so much going on in her life and in her mind personally that makes her depressed to the point where she does not want to do anything but lay around.
